This policy brief proposes a paradigm shift: moving from a reactive to a proactive system, capable of integrating clinical, social and digital data, and accompanying citizens throughout their health journey. The recommendations emphasise the need to innovate the organisation of services, enhance the use of technology, support digital inclusion and promote prevention models that reduce inequalities and improve the quality of life of older people. Personalised prevention emerges as a strategic lever for strengthening the sustainability of welfare systems and promoting active and independent ageing.
